Letter of Authorization (LOA)
Required documentation for the port request

Each carrier has their own LOA form - use the correct one

Must be signed by account holder or authorized contact

Leave no blanks - incomplete LOAs will be rejected

Sign and date the LOA

Signature must match the authorized user on the account

Include copy of valid ID

Some carriers require photo ID of the authorized signer

Keep a copy for your records

Store digitally for easy reference if issues arise

Timing & Coordination
Plan the port to minimize disruption
Request port during low-traffic period

Avoid Monday mornings or month-end - schedule mid-week if possible

Notify stakeholders of timeline

Let team know when brief service interruption may occur

Set up temporary forwarding (if possible)

Technical Setup
Prepare your new system before the port completes

Set up voicemail greetings

Record professional greetings for all mailboxes

Install/configure desk phones or softphones

Hardware should be ready before go-live

Post-Port Verification
Verify everything works after port completes

Test voicemail functionality
